Tug/pull on the tube inserts. Grommets fit/seal areas look wet. Do they feel loose? Do they semi-spin?

What brand are they? I would try a different set. Seal any mating area with Permatex sealer.

Use this one on sealing/mating surfaces. It’s my favorite for oil sealing gaskets.
