I haven't touched the CCV system for a while, I replaced the whole CCV system in 2017 shortly after I bought the car.

Yes, reset adaptations with my Foxwell several times.
Any inkling that this could just be a failing CCV all along? It has been 40k miles since I replaced the CCV now. Haven't seen any real mayo in the vacuum lines but that doesn't mean the CCV itself is not clogging up, maybe? I did see a low and unstable idle at times over the past 2 years.

The problem will be more that the diaphragm has ripped in the CCCV not so much clogging. Have you checked running with the cap off ? Did you take it in for smoke test/diagnosis?

Today I started it up and pulled the oil fillter cap off. The idle drops a bit when you do this (at least when cold). I put a plastic bag over it and it tried to suck it in. I then tried the same thing with the dipstick tube, and it also tried to suck the bag into the dipstick tube.
I also tried another test I found on bimmerfest: "With the engine off and cold, carefully remove the hose at the valve cover front corner. Blow hard into the hole. You should hear oil bubbling in the oil pan. If you don***8217;t hear the bubbling the top or bottom hose is likely cracked. The bottom hose often breaks just below the valve connection. There can also be cracks in the other two hoses." (post 4: https://www.bimmerfest.com/threads/c...system.445403/ ) When I do this last test I do not hear any bubbling. This would be hose #2 in the RealOEM diagram: https://www.realoem.com/bmw/enUS/sho...diagId=11_2194 I don't see why this hose would cause bubbling when you blow into it though. Am I wrong? The original post came from an E39 forum so I don't know if our M54 is exactly the same as theirs.

The diaphragm controls manifold vacuum to the crankcase and it's a "closed" system so any crankcase leak is also vacuum leak. If the diaphragm ruptures you get excessive vacuum "leaking" to the crankcase if you want to look at it that way.

Looks like I need to get in and start taking everything apart. Either that or drive it down to the indy to pay for a smoke test. None of the mechanics I have called have answered or called me back yet. I live in a HCOL area with lots of rich people who drive german cars so I am guessing they are all pretty busy.
