Had the same issue.

Possible Problems are:
- low freon charge
- HVAC mixing valve (also called Water Valve)
- component where the valves are installed (I don't know the name)

In my case:
- recharged the system and issues came back.
- replaced water valve and the issue came back.
- replaced charge valves assembly and all good.

Like everyone suggested start with recharge first.
