I think you are right.I remember when i was going to take the EGR cooler out while doing thermostat,I found the same thing.One of the flange bolts was totally loose.At that time i thought that someone might have left it loose un torqued.So looks like we should re-torque it again.Will check mine.

Yes, I know this is an old thread, but I'm in the process of replacing my thermostat now. When I removed the EGR cooler, the lower flange bolt was only hand tight. 2013 with 23,000 miles. This EGR cooler hasn't been touched since new.
Per the pdf instructions linked in post #1, we're supposed to tighten bolts 3, 4 & 5 to 19N-m then back off a quarter turn?
