Anything is possible to the dealer. There are also many options we don't see in U.S. catalogues that dealers can order.
With that being said. It depends how good of a friend you are with the dealer. Yes custom colors can be ordered. But unless you know the dealer on a personal basis, or have bought 8-9 consecutive BMW's from them, they probably won't do it. Under U.S. law they can't
force you to buy the car. The last thing a dealer wants is a bright yellow X5 sitting on their lot.
You'll find dealers will be a lot more flexible with interior/exterior combo's. For example, our 04' X5 is sterling grey on the outside and beige on the inside. According to BMW's catalogue for that year, thats an impossible combination, but the car sitting outside my house is proof of that.
The exception to this is if your ordering an M car. BMW dealers usually are pretty flexible with M3's, M5's & M6's.

Yes it does. Is it worth the price difference, I think you should go experience that for yourself. (Go sit in a 7 series, since some of them are specced with Napa leather).
