Actually, doctors in the USA do not get paid fee for service. The only industry where big business insurance company and stinking government don't pay their fair share. The lawmakers are all bought off by big businesses to support the most insane ways of not paying doctors....
What field do you know that people get paid a percentage of the fees in 3-4 months (if at all). There is so many rules and regulations in getting your payment. The only way doctors survive these days is to be part of a huge group. We have to pay people to get our money these days.
Next time you see a frustrated physician, you'll see why. Capitated healthcare will not survive. The big businesses have been raping the healthcare system and now the consumers are paying for it. The typical CEO of insurance companies make as little as $5 million/yr to $30 million/yr (not counting their stock options). Thus, the quality of healthcare in the U.S. will further suffer and really, what is more important these days--hopefully it is the health of you and your family.
As expensive as pharmaceuticals are, the health insurance company and big businesses are the ones that are responsible for our healthcare crisis in the U.S.

When an artist like Dave Matthews puts out a record, he has a lot of financial liability. He gets an ADVANCE from the record label which is spent on Producers, studio-time, music video's, etc. etc. which all cost a ton of money. The "Advance" is not a signing bonus, it is a amount of money that he is expected to pay back to the label for expenses. So, if he hets a $250,000 check to make the album, he owes the label that money back. And, if his work is stolen, he is responsible for coming up with it.
Now sure, we can say, but rock-stars make all this friggin money !! Well, they deserve too. They have a higer risk/reward than the average person, have suffered for their talent with no guarantees of success and being in the limelight requires them to live life a little differently.
Now, if the DM Band has to copyright their work to keep the file swappers from stealing their Intellectual Property, so be it. Is it a nuisance for people like Kevin and others, sure, of course it is, IT SUCKS to be honest..
But you can't blame the band. Blame the file sharers.
Otherwise it's like blaming BMW that we have to have alarms on our cars that we need to turn on and off because of car thieves and it's bmw's fault.
- And, let me just state, the RECORD INDUSTRY needs to do WHATEVER it can right now to protect itself. As will the film industry in the next few years. If it doesn't, the industry will cease to exist as we know it. So unless we want to sit around listening to Jethro (not TULL) playing Dueling Banjos by the fire, we must accept the fact that it is a business that is just trying to survive at the perils of theft and nothing more.
