Internal mechanism may be jammed.
Rollers may be worn
Motor gears may be stripped.
Children may have 'posted' something inside the nav

You can try any electronics shop that has experience repairing DVD mechanisms, or send to the VDO factory in Germany, or buy a replacement new mechanism from the guys in Holland

Or get another nav (that's what BMW do)
