I suspect the 03 you are seeing references the year (the MkIV's were introduced in October 2002 on the model year 2003 X5's).
The only negative on getting an older MkIV is the firmware on the newer units is reported to support Sirius. So if you are thinking of getting Sirius at some point (which will also entail replacing your radio module with one from a 2004 or newer) then you may want to get a newer MkIV. Other than that they are the same.
