What strikes me most is the per capita spend on health care, with little apparent benefit in increased life expectancy.

Some of the potential solutions seem rather obvious (like reducing or eliminating the deductability of home mortgages, which arguably leads to people speculating on homes instead of buying them to live in) but it is difficult to see where the hard decisions will come from to push solutions like that ahead.
