E53 x5 3.0D sport gearbox fault please help

Hi,
ive got a 2003 3.0D sport and seem to have a gearbox fault it seems to hold on to some gears sometimes and rev high before changing then other times it can just change gear with no reason on a flat road or change up when going down a hill.
