in many instances, i have seen first had on shocks that are bent, which can cause the rotors to become "warped"

No such thing as warped rotors.

Read that link that motordavid posted.

You probably never properly bedded the rotors in the first place. So you are getting messed up deposit buildup on your rotors. Get the rotors resurfaced, put in new pads, and bed it in like you are supposed to. Again read that article, all there.

brake dust

lets see You have four wheels and three are clean no brake dust one has turned black from wearing the pad its either stuck on or not moving back in the caliper ,hence wearing out the pad and rotor ? as the caliper gats older it starts and sticks and wears ur pads out quicker then the second time (if You just throw pads on it they wear out quicker)
