P0040 etc

Tried the search but didn't find any for this code.

1.) P0040 O2 Sensor Signals Swapped Bank 1 Sensor 1 / Bank 2 Sensor 1

This code pops out whenever I get the Engine Failsafe message.

does it literally mean the 2 sensors were interchanged?

Btw, it comes along with other codes:

2865 CDKDVPMN - Power supply limit VVT-emergency
2832 CDKPLLSU2 - plausibility diagnostics LSU by LSH after catalyst bank2
P2196 O2 Sensor Signal Biased/Stuck Rich (Bank 1 Sensor 1)

TIA !
