"Lean Run Bank 2" Diagnostic Results / Does Not Start in Cold Weather
I took my 2006 BMW X5 4.4i (N62) to the dealership to get a diagnostic test done. They came back with the results "has lean run bank 2" The dealership says there is probably a vacuum leak and it needs a smoke test for them to find out where.
We have problems with the vehicle starting when it's cold out (starts fine when it gets up to 50 degrees or so).
I was wondering if anyone else had this issue and what it might be?
I have had the spark plugs replaced, replaced the crank case vent valves, and replaced one hose on the front of the crank case. We did not replace the hose on the side that bank 2 is on. Replacing that hose on the bank 2 side is my next step but I was wondering if anyone had any suggestions beyond that?

UPDATE: Also, when the car is running (finally got it started this morning by heating the garage) there is a high pitched long consistant "beeeeep" like the high pitched sound from an ohm meter. I don't know where it is coming from but I also discovered that when the car is in 'on" position before being cranked I hear this same high pitched noise. Don't know if it is related to my above problem?
UPDATE 2: Now it's giving me the message "engine fail safe program" - I've never seen this message before.
