uphill judders

After my car has warmed up it occasionally judders while going uphill.

Also the rev counters bounces around about the 40-50mph speed.

What could be causing this?

Ok
Start with basics tune up, plugs, check coils for over heating marks and replace them all. Clean injectors, and clean the throttle body
Oh and replace the fuel filter not done already

I changed the spark plugs 11 months ago. Scanner shows no codes.

I was thinking it may be transmission related? Should I not change the trans oil and filter?

Car is at 115k miles and I dont think it has been done.
