Rear AC/Heat vent doesn't work as expected

I have manual AC system in 2003 X5
Sometimes (most times) rear vent at rear seat would blow hot air also AC is ON. What is the problem? Does anybody know?

As far as I know my system is manual, flaps controlled via bowden cables.
