Exclamation ICV Error codes

Hi all,

I just want some clarification so I'm 100% sure I'm thinking correctly while diagnosing this problem.

I have the infamous trifecta and CEL pop up, the engine failsafe mode came on too. I scanned the car using INPA and got the following codes:
211 - ICV stuck
227 - Fuel trim (bank 1) at limit
228 - Fuel trim (bank 2) at limit

I popped off the airbox tract to get to the ICV. It wasn't extremely dirty, and I could hear it rattle inside. I cleaned it anyway, checked for any holes in the intake boots, and put it back together. I ended up with the same codes after 150 miles of driving. The MAF is 2 years old, the DISA is around 6, and the intake manifold is free from any oil. Could the ICV still be "stuck" even though I hear it rattle around?

Before I drop $150 on a new ICV, I'd like a second opinion on what you guys might think this is.
