Rich and lean codes at the same time? P0171, P0174, P1093 and others

I have had an ongoing issue with my check engine light codes since I've had my X5. It's a 2006 3.0i with ~135K on it.
I have replaced the CCV system, cleaned the MAF and ICV, etc. The codes come and go. It normally was just the lean codes, but now I'm getting the rich codes too and sometimes some others. It is almost always now a P0171 P0174 and P1093 always on.

The combination and rich and lean baffles me. I haven't had any driving issues or loss in power, fuel economy, etc. I just don't know where to start looking, but I will have to take it through emissions this summer for the last time and need to get it fixed before then.
Could it possibly bad/dirty injectors or something like the ICV or MAF? I have zero vacuum leaks and drive with the status monitors up. My fuel trim on both banks is consistent as well as my voltage readings on all 4 o2 sensors.

Help?
