8D Activation, electric fan

I continue to get this error code 8D on the scanner, and the electric fan never seems to run.

The engine runs normal temp. When checking the data stream on my iCarsoft, the Voltage, temperature sensor, engine outlet is always 0 volts. I need to check if the fan runs by hot wiring, but could this be the engine temp sensor is bad? And where might that sensor be located . I also always see the Duty cycle for the electric fan stays constant at 8%. Curious for a fan that never operations. Any help here appreciated at I continue to diagnose. I just didnt want to replace the fan assemble if its a sensor or actuator that is really the problem.
