P0306 When Starting Cold

2006 3.0 with 67,000 miles. When I start cold I get a p0306 misfire cylinder 6 code. I changed the coil pack, I changed the spark plug. I still got the code with cold starts. I swapped the coil packs and plugs around from cylinder to cylinder and the problem stayed with cylinder 6. If I shut it off and restart it goes away, sometimes if I rev it, it goes away. Then I got a p0365 crankcase position sensor B bank 1. I replaced the sensor and the code cleared. I still get the misfire on cylinder 6 with cold starts though. I took the DISA off and inspected, it seems ok. I see no leaks in the intake boots. Any ideas? Thanks!

Thanks for the link. I too hope that I don't have to pull the cylinder head off. Could you see the failure in your gasket when you took the head off?

Nope, not in the gasket or sealing surfaces of the head or block.
But the tops of 2 pistons were steam cleaned and and looked brand new. The other 4 had some carbon/sludge build up
