DISA Valve bad???

I had "service engine soon" light on so had it diagnosed at a shop ( Not a BMW indie) standard shop. They gave me a list of stuff I needed Crack Case Valve,Breather hose,Vacuum hose,Thermostat,Intake Elbow Boot etc....

Question 1: I had my DISA Valve (Intake runner valve) replaced 2-3 years ago, How would it be bad already? I pulled it off and seemed to be sturdy and the butterfly valve was engaging from what I saw. How do you reallly know when its bad?

Question 2: I can not find the location of the crank case valve. Anyone know??
