Trans failsafe Please HELP

Dear Fellow X owners.
Desperately need help. I have run thru many forums and would still need some expert input.
I have two two major issues i need help with my e53 3.0i

1. Recently had my transmission overhauled due to failure. Changed all clutches, solenoids, filter, replaced fluids and reconditioned torque converter. Trans failsafe mode error comes out while cruising at 80 to 90 mph. However, it is still able to drive normally and change gears up and down. Also occasionally does not change from 1st to 2nd gear until revs hit bout 4000 rpm while light on throtle. Error that is indicated is converter lock up excessive slip. Have so far changed converter lock solenoid, battery as well as alternator after reading numerous forums. Desperately need help as i am running out of possible solutions.

2. Another issue is error code P0161 which is sensor heater faulty bank 2 after cat. Have replaced the o2 sensor but problem persists. Also switched the wiring with bank 1 but error code still there.

Did you replace the Torque Converter when you had the trans replaced? Did you flush the trans cooler in the radiator when you rebuilt the trans? Debris from a bad trans made have gotten into the torque converter and trans cooler.

Thanks upallnight. No..did not change the converter. Just got it reconditioned. Will give that a shot and keep you updated.

Mistasteppa. My wheels and tires are new.. barely 3 months. May not be the solution for me.

Make sure the harness connector to the transmission is clean. There are a lot of pins in that connector, and I was told it was not uncommon to find factory harnesses lacking di-electric sealing grease in those sockets. Mine looked like it was packed with mud when it was taken apart and it made the transmission act up like crazy.

What happens if you put the trans in manual mode and take off in first and second gear?
When the light comes on at 80-90 do the revs increase normally if you continue to accelerate?
