They are basically washers that go in between the wheel and the hub. They push the wheel out further for a more aggressive look. The 20mm is the thickness of the spacer. You will also need longer lug bolts. H&R is a common manufacturer of spacers.

You do not need longer lug bolts. Spacers of this size on a heavy truck i would never run with just longer lug bolts especially the way i push mine...
The ones you want are called DRA type, i have h&r ones, they first bolt to hub with included lugs, then your wheel bolts to it.

A problem I encountered on my 25 mm spacers was that the original bolts stick out that the rims would not fit flush. I am going to have the bolts machined to remove at least 5 mm? Anybody have any ideas?

Eugene, the new studs and bolts are to bolt the spacer to the hub. Then your original lug nuts will bolt your wheel to the spacer. It looks/ sounds like you are doing it opposite. That would be why it doesn't fit.

Spacers in the photo have the studs in the spacer and require lug nuts for the wheels, not lug bolts. 25 mm spacers require shorter lug bolt head; 30 mm do not. But most wheels have recessed space between the lug holes (not flush) so lug head sticking out slightly shouldn't matter. Also, the spacer in the picture doesn't appear to have hub-centric lip. Highly recommend you get spacers with hub-centric lip to minimize vibration.

I was also going to mention the hub-centric lips missing from the spacers pictured. There is no way I would attach spacers without them, as extra load is placed on the wheel nuts and bolts, and there is no way the wheel can be properly centralized without them. Just relying on the bolts to do this is not satisfactory, and can cause vibration at any speed.
