The FAILSAFE message would appear upon starting the engine before even touching the gear lever.
One other thing I failed to mention was that once I started getting the message, the shift from Park to Reverse was rather abrupt, not smooth like when the transmission was acting normally. It certainly will be interesting to see how the transmission acts once I get everything back together again!

Haven't had a chance to work on this project more lately, but my 20Kg weight did show up today. Maybe this entire trans teardown will have been for nothing, but at least I'll have piece if mind when it's all back together!
 It's bigger and heavier than it looks! That's a 39" 4K monitor in the background! And yes, the mouse pad lady is well endowed, although nothing compared to my wife!
