I have to use the phone to switch to the handset. I was unable to find any other way with my Blackberry 7520. On my phone you push the side wheel to bring up the menu, select "Activate Handset" and press the wheel again. Check your phone's instructions.

From what I know, the BMW BT is always on, and so the only way is to turn off your BT on the phone.

For the Motorola Vxxx, you have to perform it on the phone. Select Settings, Connection, Bluetooth Link, Handsfree and then drop the active connection.
