Yes the X5 was purchased used but there never was a CD-changer in it. The 3 and 6 pin plugs were found bundled unused among the wiring.

BUT here's what I found... the DSP amplifier HAS A COAX terminal on it - although nothing is attached... does this mean I need a Digital DSP adapter even if I have the 6 pin plug?

It's likely you have a Digital DSP setup, requiring the adapter. Here's the item you need:

DSP Kit for DICE iPod Integration Kits

This will likely be the missing link to get your audio back.

Mine did not have a CD changer in it. I doubt that the cable in back of the radio is unplugged on mine - BSW sold me an XM radio kit with a blitzsafe as well as the Dice module. The XM radio works great - I get great sound with it and the XM works with the radio and steering wheel controls. I have a feeling that there may be a bad cable between the blitsafe and the Dice? My DSP unit does not have the coax cable coming out of it, but I do see a coax "female" connector on the upper surface of the DSP unit. So, does this mean that I also need a DSP adapter?

Sirius (which is what the XM Direct is emulating) has nothing to do with DSP. As a test, you can try removing the SLD01 interface and plugging the DICE in directly to see if audio is achieved.

I did try that - and I still have no audio. When I plugged the Dice in directly (when I removed the XM and blitzsafe), I had the same situation - scrolling text on the radio, but no audio.

You've just confirmed a Digital DSP setup. You'll need the DSP adapter.
