Our house rule is we don't do surveys, make purchases or give out any information over the phone unless we initiate the call or know the person on the other end. The only exception to the surveys is when BMW calls to follow up on a recent service.

The CC companies really should print a warning on the back of the card about giving out the verification code over the phone.

Totally Agree.
And everyone should have a paper shredder for anything printed with acct #s, etc on it, even basic stuff. There are many services as simple as local utilities that will give out info (often mistakenly) based on a "customer call" with address, name & utility (for example) account #. Cell phones co's are getting a lot of flack too for sharing too much info. When in doubt, put a cross-cut shredder to work next to the recycling bin!
