Engine runs rough when cold

When cold my 4.4L V8 runs at a lower idle so that the whole vehicle has that low hum slight vibration and everyone 10 seconds or so the engine sort of misses. Fuel pump? Fuel filter? Spark plugs?

You shouldn't use tap water in the cooling system at all, if you want to do a complete flush you should drain it from all drain points (radiator plus engine block)...
