There are 2. 1 in the times square area, and 1 on the upper west side. Just google Carmines NYC and you'll get their info. Carmines used to be a lot better, but they've gone downhill the last few years. They don't take reservations unless you have like 10 people and even then you'll be waiting forever as the tourists have pretty much taken over both restaurants.
IMHO, if you want AMAZING Italian in midtown, Patsy's (not patsy's pizza in the village) on 56th is the place to go. The menu is huge, the wine list is huge, the service is top notch, and the food is outstanding. You don't even need to look at the menu if you don't want to, you can just tell your waiter what you want and most of the time they'll make it for you.

There are other very good italian joints in NYC, don't get me wrong, Little Italy didn't get it's name because it's shaped like a boot, the restaurants down there are also very good (monty's comes to mind). Anyway, check out patsy's, you won't be sorry.
