Rome is different. It is not just with the Vatican, but with any small religious site. Churches in San Gimignano, Firenze, Siena, Pisa, Montepulciano, Pienza, and others all required covered shoulders when we visited (usually via a borrowed shawl) for entry. Shorts on ladies were frowned upon, but were tolerated by some staff and not others. Realize that it is their country, that these are actual churches and not tourist attractions, and that modesty counts, and you will be fine.

For us a tour was the very best. We saw a lot, stayed in the best hotels.
ate the best food and everything was paid for. I mean everything including
tips, meals, luggage transfers. It was a blast and we saw all the point's of interest
you mentioned and got a real education. Of course that was a few years ago but WOW what a trip.

You can hire an automatic car no problem. A Cooper S Cabrio auto would be nice for you. They are some cracking roads for a the mini in Italy.

You could always do the Italian Job in Rome we could hear about it in the news.

The other tip I would have is to walk in Rome as much as possible it is amazing what you can happen upon just by wandering.
