Yes, if they change the resulting wheel offset from what it is intended to be. Also bad for suspension components which are subjected to larger loads. And wheel bolts/studs. And if installed on the front wheels, steering geometry.
But they make the tires look wider, which is why people buy them, despite the above.

They cause all those issues, apart from putting additional stress on the wheel bolts (as long as they are bolted spacers, hubcentric, and don't just use longer bolts). And there, they double the number of failure modes, making a problem statistically more likely.
